Where can we obtain the test requisition form?

How do we order Belay shipping kits?

What are the specimen collection instructions?
Collect >=6 mL of CSF in a standard CSF tube. This is the minimum required sample volume for performing the Summit and/or Vantage tests individually or combined. Please label tubes with collection date and include two unique patient identifiers (e.g., patient name, date of birth, medical record number). We recommend reviewing our detailed CSF Sample Handing instructions.
What are the transport temperature requirements?
Maintain CSF sample at room temperature. Do not freeze or refrigerate.
What are the shipping instructions?
Specimen must be shipped within 24 hours of collection and received at Belay Diagnostics within 48 hours of collection. Send sample in a Belay shipping kit along with the completed test requisition and proper informed consent form. Customer Service can email a FedEx label to your email address and schedule a pickup for you. Please call Customer Service at 331-320-0155 for assistance.
Can we use our own collection tubes?
Cerebrospinal fluid specimen can be collected and transported in a standard CSF tube.
Can a specimen type be sent other than CSF?
Summit™ and Vantage™ tests investigate tumor derived DNA extracted from cerebrospinal fluid only.
Do you accept blood-tinged CSF samples?
Every specimen is vital to the patient, clinician and Belay Diagnostics. We will do our best to report on every sample, even if blood-tinged.
My patient just had a lumbar puncture on Friday at 4pm. How can I get the sample to Belay?
If you are a current Belay Diagnostics customer, please ensure you complete the test requisition and enclose it along with the informed consent and sample in the Belay shipping kit. Please call Customer Service at 331-320-0155 to assist with scheduling a pick-up.
What should we do if our pick-up was missed?

What is the BelayAccess™ (BAP) program?
We’re committed to providing advanced testing for all eligible patients. We provide self-pay options for patients and tiered pricing based on income. Health Savings Account (HSA) and Flexible Savings Account (FSA) apply. For patients with out-of-pockets costs, financial assistance is available for those who qualify. Are Summit™ + Vantage™ covered by my patients’ insurance?
Belay accepts all commercial insurance plans. Patients’ out-of-pocket costs may vary depending on their plan benefits. Please see details on BelayAccess Program (BAP) above to see how we can help with costs.
Does Belay Diagnostics offer financial assistance to patients?

Will I be responsible for obtaining my patients prior authorizations?
No, Belay will manage submitting prior authorization on your behalf. If a physician’s assistance is required, our team will reach out to your team directly for assistance.
Will I be responsible for the appeal process if my patient’s claim is denied?
Belay will manage submitting all appeals on the patient’s behalf. If a physician’s assistance is required for a peer-to-peer, our team will reach out to your team directly for assistance. Belay’s goal is to manage all billing issues for physicians and patients to remove any type of burden.
Does Belay accept Medicare and Medicaid?
We currently do not accept Medicare or Medicaid. Our team is working diligently to gain Medicare and Medicaid coverage. What happens if my patient does not have insurance?
Belay Diagnostics believes every patient should have access to our testing, regardless of financial situation. We offer financial assistance for all patients who are eligible. To apply please visit BelayAccess Program (BAP) above.
